It occurred to me a little while ago that if I suddenly died, went into a coma, or was otherwise incapacitated, it would be quite the scramble for family/friends to figure out Axxle’s care. You know, his medication schedule, what meds need refilled or renewed when, who all his specialist and therapies are, when his next appointments are scheduled…all the important details of caring for a medically complex child.
No doubt they would eventually figure it out…I mean I do have his MyChart on my phone. But it’s something that I should probably make quickly and easily accessible for their sake AND Axxle’s. And I really should have done this years ago…medical mom fail on my part…
Because it finally came to my attention, I decided to create an emergency, medical folder to put all of Axxle’s important medical information in. I know they make great apps now for this kind of thing, but being an elder millennial, I prefer old-school pen and paper.
I want to share this with all the other families out there who could use it too. Totally free because medical life is already expensive enough.
What’s included in the medical planner download:
- Emergency Info for a quick summary
- Specialist/physician list
- Medication/pharmacy information
- Medication tracker
- Daily planner page
- Monthly planner page

I printed it, filled it out, and put it in a binder in a central location. And of course let my family know it was there, just in case.
All you have to do is click the link, download and print!…and share with all your medical fam friends 🙂 Tip: laminate the medication tracker and use a dry erase marker so you can reuse it every week.
And if you’re in need of a hospital packing list for your child, I’ve got one of those available for download as well. You can check it out here.
